NOTICE is hereby given pursuant to the provisions of Section 414-387 of the Hawaii Revised Statutes, as amended, that THOMAS H. MAEDA, JR., M.D., INC., a Hawaii professional corporation, has filed its Articles of Dissolution with the Department of Commerce and Consumer Affairs of the State of Hawaii. All creditors are hereby notified to present their claims with proper vouchers or duly authenticated copies thereof to the corporation at the following address: 4389 Malia Street, #131, Honolulu, Hawaii 96821. Each claim must include the following information: name, address and phone number of the claimant; the amount claimed; the date on which the claim arose; the basis for the claim; and documentation for the claim.
