NOTICE IS HEREBY GIVEN that C&Y CPAS LLC, a Hawaii limited liability company (the “Company”), has been dissolved and intends to terminate its existence. All persons with claims against the Company are required to present their claims in writing, including the claimant’s full legal name, address, phone number, summary description and amount of the claim, and date the claim arose to the Company at c/o Accuity LLP, 999 Bishop Street, Suite 2300, Honolulu, HI 96813.

Claims against the Company will be barred unless a proceeding to enforce the claim is commenced within two years after the last date of publication of this notice.
