VARIANCE APPLICATION NO. WW 765

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Ms. Karin O’Mahony of Saddle City Ohana LLC has applied for a variance for the maximum of five (5) years from section 11-62-31.1(a)(1)(D) of Hawaii Administrative Rules, “Wastewater Systems.”
Mr. Michael McNulty, P.E. is the authorized agent to act for the applicant. The variance request is for the continued use of an individual wastewater system located at 41-363 Saddle City Road, Waimanalo, Hawaii, 96795 and TMK (1) 4-1-013: 034.
