VARIANCE APPLICATION NO. WW 747

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person regarding the following. Mr. Kyle Spencer has applied for a variance for the maximum of five (5) years from sections 11-62-33.1(b)(6) and 11-62-34(c)(1)(C) of Hawaii Administrative Rules, "Wastewater Systems."
Mr. John Cabulisan of JBC Engineering and Construction, LLC is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 67-251 Kukea Circle, Waialua, Hawaii 96791 and TMK (1) 6-7-007: 013.
