NOTICE IS HEREBY GIVEN that Grant T. Miyashiro, O.D, LLC, a Hawaii limited liability company (the “Company”) will be dissolved pursuant to unanimous consent of the member of the Company. All claims against the assets of the Company must be made in writing and include the claim amount, basis and origination date. The deadline for submitting claims is December 15, 2021. Any claims that are not received by the Company prior to the date set forth above will not be recognized. All debts owed to the Company, and all claims against the Company, will be received by Seth T. Any claim against the Company is barred unless a proceeding to enforce the claim is commenced within two years after the later of the last publication date of this notice or the date the Company files its Articles of Termination with the State of Hawaii Department of Commerce and Consumer Affairs.
