Pursuant to Hawaii Revised Statutes Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested person(s) regarding the following. Mr. Ryan Frederiksen has applied for a variance for the maximum of five (5) years from Hawaii Administrative Rules, Chapter 11-62.
Mr. Dennis Poma, P.E., ACSI Inc., is the authorized agent to act for the applicant. The variance request is for the construction of a new individual wastewater system to be installed at 609 Milokai Street, Kailua, Hawaii 96734 and TMK: (1) 4-4-039: 007.
