NOTICE OF DISSOLUTION

Notice is hereby given that Surgical Associates, Inc. (the “Company”) has dissolved. All persons with claims against the Company must present them in writing to: Surgical Associates, Inc., 550 S. Beretania Street Suite 403, Honolulu, Hawaii 96813. A claim against the Company will be barred unless a proceeding to enforce the claim is commenced within five years after the publication of this notice. Any claim against the Company must include the claimant’s full legal name, address, and telephone number and a description of the amount and nature of the claim including the date the claim arose.
