WASTEWATER SYSTEMS

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. D. G. Anderson of Honolulu, Oahu, Hawaii, Manager of A6 LLC dba Haleiwa Beach House has applied for a variance for the maximum of five (5) years as stated in the variance application from Hawaii Administrative Rules (HAR) section 11-62-31.1(a)(2)(D).
The variance request is for the continued use of a septic tank system that servces the Haleiwa Beach House restaurant that is located at 62-540 Kamehameha Highway, Haleiwa, Oahu, Hawaii 96712 and TMK (1) 6-2-003: 014.
