NOTICE OF PROPOSED WATER POLLUTION CONTROL PERMIT FOR LONO KONA SEWER IMPROVEMENT NPDES PERMIT NO. HI S000530 DOCKET NO. HI S000530 December 7, 2017 The State Department of Health (DOH) tentatively proposes to issue a National Pollutant Discharge Elimination System (NPDES) permit to discharge storm water associated with construction activity from the Nuuanu Reservoir No. 4 Dam Improvements project site, to receiving State waters, subject to special conditions to: CITY AND COUNTY OF HONOLULU BOARD OF WATER SUPPLY The proposed permit for the new discharge will expire five (5) years from the date of issuance. The project is located at a portion of 4161 Nuuanu Pali Drive, 0.25 miles northeast of the intersection at Nuuanu Pali Drive and Pali Highway, 4 miles northeast of downtown Honolulu on Pali Highway, State Route 61, Honolulu, Island of Oahu, Hawaii, TMK: (1) 2 002 054. The primary purpose is repair of the reservoir and dam, however, new structures will also be built including a concrete stilling basin, access road resurfacing, v notch weirs at the seepage areas near the riprap slope, and extension of an existing 24" outlet pipe. Construction activities include installation of erosion control measures; clearing and grubbing of the staging area; preparation of a sediment/settling basin; dredging, excavating and stockpiling of the reservoir; repair of the intake tower and bridge; construction of weirs, a stilling basin, and an outlet pipe extension; grading and mulching of the settling/stilling basin and replanting of trees and vegetation; resurfacing the access road; grading and restoring the downstream embankment; installation of permanent stabilization measures; and removal of erosion control measures. The receiving waters, Nuuanu and Lulumahu Streams are classified by the DOH as a Class 1, Inland Waters. It is the objective of Class 1 waters that these waters remain in their natural state as nearly as possible with an absolute minimum of pollution from any human-caused source. To the extent possible, the wilderness character of these areas shall be protected. Persons wishing to comment upon or object to the proposed NPDES permit or to request a public hearing, should submit their comments or requests in writing no later than 30 calendar days after the date of this notice, either through E-mail at cleanwaterbranch@doh.hawaii.gov or by mail at P. O. BOX 3378, Honolulu, HI 96801 3378. Copies of the proposed public notice permit and other information are available for public inspection, Monday through Friday (excluding holidays) from 7:45 a.m. until 4:15 p.m., at the DOH Clean Water Branch office located at 2827 Waimano Home Road, Pearl City, HI 96782. If DOH’s position is substantially unchanged after considering all timely written comments and all oral comments at any public hearing that may be held, then the DOH will issue the NPDES permit and this action will be final.
