NOTICE OF INTENT TO TERMINATE

ALOHAFNP CONSULTING LLC, a Hawaii limited liability company (the "Company"), intends to terminate effective June 28, 2019.
In accordance with HRS § 428-808, all persons with claims against the Company are notified to present them in writing to the Company, c/o Tina Tsuchiyama, Carlsmith Ball LLP, 1001 Bishop Street, Suite 2100, Honolulu, HI 96813.
Claims must include: (1) claimant’s name, address, and telephone number; (2) name of a contact person, if claimant is an entity (e.g., a corporation, LLC, or partnership); (3) dollar amount of the claim; (4) date the claim arose; and (5) brief explanation of the nature or basis for the claim.
Any claim against the Company is barred unless a proceeding to enforce the claim is commenced within two years of the later of the last publication date of this notice or the date of filing of the Company’s articles of termination.
