NOTICE TO CONTRACTORS SEALED BIDS will be received up to and publicly opened and read aloud at 2:00 p.m. on Friday, October 13, 2017, in the Administration Conference Room of the Waianae Coast Comprehensive Health Center, 86-260 Farrington Highway, Waianae, Hawaii 96792 for: WAIANAE COAST COMPREHENSIVE HEALTH CENTER MALAMA RECOVERY BUILDING PROJECT – LOWER LEVEL BUILDOUT 86-260 Farrington Highway Waianae, Hawaii 96792-3199 TAX MAP KEY: 8-6-01: 03 The project consists of providing all materials, labor, equipment, etc., to accomplish construction work consisting of, but not limited to site work, mechanical remodel, plumbing, mechanical, electrical and minor renovations to the existing building which the plan is to enclose the understory of the footprint and include addition of exterior decking and waiting area. Plans and specifications will be available from Monday, August 28, 2017, at HonBlue, (ph. 531-4611) at Contractor expense. This project is partially funded under: Title I of the Housing and Community Development Act of 1974 (Public Law 93-383). Contractor will be required to comply with all applicable current State and Federal Labor standards and Davis Bacon Regulations at the time the Contract is executed. To be eligible to bid, the prospective bidder must provide written notice of bidder’s intention to bid. Bidders shall be licensed in accordance with Chapter 444, HRS, relating to the licensing of Contractors. The Owner’s Representative must receive such written notice by 2:00 pm on Friday, September 22, 2017 via email submitted by PDF to lmahoe@wcchc.com, cc: jgriego@wcchc.com. The notice shall provide the following information: 1. Name of Contractor; 2. Project title, and description; 3. Bid opening date: 4. Contractor’s license number, and 5. Contractor’s address, telephone, and facsimile, and email address. A pre-bid conference and site inspection will be held on Tuesday, September 12, 2017 at 10:00 a.m. to meet at Malama Recovery Building Conference Room. This will be the only opportunity to visit the site during the bid period; therefore attendance at this meeting is encouraged. All bids shall be accompanied by a bid security of the amount not less than five percent (5%) of the bid amount. Bidder’s attention is directed to Act 352, SLH 1997, relating to the requirements for tax clearances from the State Department of Taxation and the Internal Revenue Service, as a prerequisite to entering into a contract and final payment of a contract.
