NOTICE OF INTENT TO TERMINATE
Genesis LLC, a Hawaii limited liability company (the “Company”), was dissolved, intends to terminate, and requests persons having claims against the Company to present them in accordance with this Notice.
Each claim against the Company must include: the name and address of the claimant, the amount claimed, the basis for the claim, and the date(s) on which the event(s) on which the claim is based occurred. Each claimant is to send its written claim to the Company at its mailing address:
