Notice is hereby given that this Mechanics Lien, this "Lien" is filed as of May 21, 2019, by GAR Corp. dba Global Auto Repair located at 231B Sand Island Access Road Honolulu, Hawaii 96819, (the "Claimant") claims a mechanical lien in the sum of $4626.33 for parts, labor, and services furnished, owned by Kaleo Carvalho ("the Owner”) located at GAR Corp. dba Global Auto Repair located at 231B Sand Island Access Road Honolulu, Hawaii 96819, with the legal description ("the Property") 2000 Toyota Corolla VIN# 1NXBR12EOYZ344389, Lic.# SZT-241 HI.
The claimant and the owner entered into a contract on 5/18/18 for repairs to be completed under an insurance claim, with owner being responsible for his agreed upon $500 deductible. Repair was completed and owner was notified 5/30/18.
As of May 20, 2019, the claimant has received payment of $4224.33. Total Balance due from Owner is $400.00.
The Owner has failed to pay the Balance Due despite many demands and requests for payment.
Accordingly, the Claimant declares that claim amount of $400.00 is justly due to the Claimant.
The Claimant declared that the contents of this Lien are true and correct to the best of their knowledge.
