NOTICE TO BIOLOGICAL MOTHER

To: Nellie Futi, Biological Mother of I.K.T.T.

You are hereby notified that on October 2, 2006, a male child was born. You have been named and are believed to be the biological mother.
A Petition for Adoption of said minor child was filed in the Superior Court of Muscogee County, Georgia, Case No. AD18DM76-78. If you are the biological mother, you are to make known any objections you may have to the adoption on December 13, 2018, in said Court at 2:00 p.m. before the Honorable Maureen Gottfried, Superior Court of Muscogee County, located at the Government Center, 100 10th Street, Columbus, Georgia.
Be advised that you will lose all rights to the child and will neither receive notice nor be entitled to object to the adoption of the child unless, within thirty (30) days of receipt of this notice you file an objection or otherwise contest said adoption, and send a copy to the below listed counsel for the Petitioner.
Be advised further that if you wish to make any claim you must be prepared to assume all financial and other responsibility for the child and prove your fitness.
